Thyroid function tests were performed on 445 in-patients over a four month period at the Department of Medicine of a teaching hospital attached to the University of Kiel. Excluding those severely ill, moribund or known to have abnormal thyroid function, the study included all patients over 60 years of age. Diagnostic criteria were clinical symptoms, results of the TRH test and of T4-RIA, in addition to nonspecific tests of blood sugar, serum cholesterol and blood count. Among the 445 patients there were 15 with manifest hypothyroidism (3.4%), previously undiagnosed. A further 23 (5.2%) had the subclinical variant of "borderline hypothyroidism". The sex ratio was normal for manifest and for borderline hypothyroidism. The classical cardinal clinical symptoms were present in only those with severe and marked hypothyroidism, i.e. not even a third of all those with manifest hypothyroidism.

The balance between food intake and energy expenditure is precisely regulated to maintain adipose stores. Leptin, which is produced in and released from adipose in direct proportion to its size, is a major contributor to this control and initiates its homeostatic responses largely via binding to leptin receptors (LepR) in the hypothalamus. Decreases in hypothalamic LepR binding signals starvation, leading to hunger and reduced energy expenditure, whereas increases in hypothalamic LepR binding can suppress food intake and increase energy expenditure.

Light is the most reliable environmental cue allowing animals to breed successfully when conditions are optimal. In seasonal breeders, photoperiod (length of daylight) information is sensed by the eyes and transmitted to the suprachiasmatic nucleus, the master clock region located in the hypothalamus. This structure has a 24-h firing rhythm involving a cycle of clock protein synthesis and degradation, and provides the timing to synchronize the synthesis and release of melatonin, the chemical signal that transduces the photoperiod information.
