The main aim and task of the investigation was to present clinico-anatomical substantiation for plastic operations with the application of flaps with the axial type of blood circulation in treatment of this category of patients. The investigation included 348 clinical cases (for exact statistical data); 65 volunteers (for the assessment of physiological parameters) and 16 topographo-anatomical preparations (variants of the vascular network of the leg). The methods of clinical and rentgenological control were used.

The authors have distinguished fascial-cellular spaces of the mediastinum, noted their developmental changes and topographo-anatomical relationships, determined possible ways of spreading the pyo-inflammatory process in the mediastinal fat in children of different age groups. Substantiation of methods of drainage in acute suppurative mediastinitis is given in the age aspect, as well as the method of local detoxication.

On the crus, the possibility to elevate two more types of the island cutaneous flaps without transection of the major vessels is substantiated. The variants of microsurgical autotransplantation and trasplantation+ of the composite cutaneous flaps of the forearm and crus on the vascular pedicles are discussed.

Based upon results of topographo-anatomical research and personal experiences with transplantation of 91 tissue complexes on radial vascular bundle the authors made an attempt to systematize the available data concerning this source of tissues. The substantiation of using the tissue complexes consisting of several fascio-cutaneous (fascio-cutaneous and muscular) flaps at different levels of the vascular pedicle as well as superlong deltoid-radial flap on two vascular pedicles are first given.
