A peritoneojugular shunt operation was done in a cirrhotic patient with ascites. The Le Veen valve functioned correctly for six months. The patient then experienced recurrence of ascites following an uncommon complication: migration of the shunt tubing into the peritoneal cavity. The tubing could no longer be felt in the cervical or thoracic regions. Plain roentgenograms of the abdomen showed that the shunt tubing was in the peritoneal cavity.

The siliconated prostheses are expensive and in the process of time complications are registered consisting of clogged valve or obstruction, requiring thus their replacement.

The case of an uremic patient receiving dialytic treatment, who also had an hydiopatic ascites, probably due to peritoneal hyper-waterproofing, is described. Peritoneo-venous shunt was positioned with good result. Generally, similar clinical situations such a therapy is as valid as other commonly used methods.

The authors report a case of a patient with a refractory ascites due to extrahepatic portal thrombosis in course of idiopathic thrombocythemia. A peritoneovenous shunt was applied and as a late complication a massive thrombosis of the intracardiac portion of the duct developed. Thrombolysis was obtained with tissue plasminogen activator at doses usually administered for acute myocardial infarction.

A case of chylous ascites due to retroperitoneal tumoral compression is reported. Following failure of a medical treatment which consisted of paracentesis and medium-chain triglyceride (MCT) diet, the ascites dried up after installation of a peritoneum-jugular vein shunt valve. This operation is seldom performed in patients with chylous ascites since the medical treatment with paracenteses, MCT diet or even total parenteral nutrition succeeds in drying the effusion in almost 50 percent of the cases, notably those with postoperative lesion.
