A simple and effective technique of retrosternal gastric bypass of the excluded oesophagus for the palliative treatment of unresectable carcinoma of the upper thoracic oesophagus is described. It was used in 60 patients, of whom 5 (8,3%) died. Postoperative morbidity was considerable, complications including chest infection in 20 patients (33,3%), wound infection in 8 (13,3%) and anastomotic leakage in 5 (8,3%); the latter caused 2 deaths. All of the patients who survived the operation were able to swallow solid food and saliva satisfactorily.

Objective: To investigate the role of endoscopic ultrasonography (EUS) in the diagnosis and treatment of upper gastrointestinal bleeding of unknown origin in liver cirrhosis, focusing on patients with recurrent treatment of esophageal and gastric varices who failed to identify the bleeding site under direct endoscopy.

Background: Esophagogastric variceal bleeding is one of the severe complications of decompensated liver cirrhosis, and serial endoscopic therapy can improve the long-term quality of life of patients. Most acute bleeding can be detected under direct endoscopy with thrombus or active bleeding, but there are still some patients with recurrent bleeding after repeated treatments, and it is difficult to find the bleeding site, especially in gastric variceal bleeding.

A 62-year-old man was diagnosed with Stage Ⅲ signet ring cell carcinoma of the lower thoracic esophagus. The patient underwent 2 courses of neoadjuvant cisplatin and 5-fluorouracil(SP therapy), demonstrating stable efficacy. Subsequently, the patient underwent subtotal esophagectomy with thoracoabdominal 2-field lymphadenectomy via right thoracotomy, followed by esophageal reconstruction using a gastric tube through a retrosternal route.

A 73-year-old man who had undergone esophagectomy and retrosternal gastric tube reconstruction for esophageal cancer 8 years prior was transferred to our hospital for the treatment of an acute myocardial infarction. Emergent percutaneous coronary intervention for the left anterior descending artery (#7) was successfully performed. However, echocardiography revealed a ventricular septal rupture (25×27 mm).

Combined therapeutic regimens, including molecular targeted agents, are considered standard treatment for advanced gastric and gastroesophageal junction cancer. We present an extremely rare case of acute reversible cardiac dysfunction in a 20-year-old patient after administration of trastuzumab plus 5-fluorouracil for the treatment of metastatic gastroesophageal junction cancer. During the first day of chemotherapy, the patient complained of retrosternal pain radiating to the scapular regions bilaterally.

Aim: This study aims to explore the rationality, feasibility, safety, and effectiveness of single-incision laparoscopic gastric pull-up (SILS-GPU) and robot-assisted gastric pull-up (R-GPU) methods.

Methods: Hospital records of patients who underwent gastric pull-up with either SILS-GPU or R-GPU between May2016 and January 2024 were reviewed. Demographics, diagnosis, surgical techniques, and postoperative outcomes were evaluated.
