Aortitis, defined as inflammation of the aorta, can lead to aneurysms and dissections. Intra-operative sampling is essential for diagnosis, with many cases presenting asymptomatically as clinically isolated aortitis. Previous studies investigating aortitis in major aortic surgery have been limited by low intra-operative sampling.

A 61-year-old man was referred to our hospital for Stanford type A acute aortic dissection and an emergency operation was planned. In this case, the aberrant left vertebral artery originated directly from the aortic arch. Total arch replacement at the level between the left carotid artery and left aberrant vertebral artery was performed and an open stent graft was deployed in the true lumen of the descending aorta to obtain better distal aortic remodeling.

Venous aneurysms are rare, and their incidence is unknown. External iliac vein aneurysms are even more rare with only 50 case reports published from 1950 to 2018. We present a case of an individual who died suddenly from a pulmonary thromboembolic event due to an external iliac vein aneurysm formed by a remote penetrating injury.

Ascending aortic replacement is a basic procedure in aortic surgery. The crucial factor is the anastomosis method, including the anastomosis site formation. The anastomosis method is usually completely different between true aneurysms and acute aortic dissection because of the difference in the fragility of the aorta.
