The authors report the results of a forensic medical investigation of 6 cases of death associated with the administration of pharmaceutical products documented by forensic medical experts of the Russian Centre of Forensic Medical Expertise. The results of the study are compared with the clinical data and summarized using the methods of tanatogenetic analysis. The following main clinical variants of iatrogenic anaphylactic shock (IAS) are distinguished: bronchospastic IAS (n = 1), asphyxic IAS (n = 1), hemodynamic IAS (n = 3), and combined (bronchospastic plus hemodynamic) IAS (n = 1).

The results of forensic medical investigations of 100 corpses of the persons who died from combined intoxication with two and more poisons are presented. They were compared with clinical observations and summarized uisng the methods of tanatogenetic analysis. The main variants of fatal combined poisoning and the underlying tanatogenetic mechanisms were categorized into cardiac (disturbances of rhythm and conductivity, progressive hypotension, flabby heart muscle, extensive fragmentation of cardiomyocytes), cerebral (coma and massive death of brainstem neurons), and pulmonary (of pneumonia- or pulmonary oedema-type with fibrin precipitation in the alveoli and formation of hyaline membranes).

Cadaverous brains of 21 men and women were available for investigation. All the subjects died in a hospital from hemorrhagic shock (HS) caused by stab injuries to internal organs and blood vessels. In 9 cases, blood and urine contained no traces of ethyl alcohol; it was detected in the blood of 12 cases (1.

Applied aspects of multivalued and modal logics served the basis for the proposed algorithm of tanatological analysis of autopsy material including formalization of semiquantitative data on changes in vital organs with further conversion of the estimated tanatogenetic coefficients in the form of probability suggestions leading to the conclusion about the kind and speed of tanatogenesis. The method is illustrated by cases from expert practice.

[Morphology and morphogenetic mechanisms of brain lesions in acute generalized peritonitis].

Arkh Patol

September 2003

Central Laboratory of Pathology, Ministry of Defence, 105229, Moscow.

Pronounced alterations of brain elements (vessels, neurocytes, glial cells, neural fibers and synaptic apparatus) are observed in acute generalized peritonitis. The following morphological forms of peritoneal encephalopathy were recognized: diffuse-focal lesions, incomplete partial necroses, inflammatory changes in the form of meningitis and encephalitis. The morphological changes were provoked by some factors resultant from developing endotoxicosis and homeostatic disorders.
