We present a case of a 42-year-old male with sarcoidosis manifesting as endobronchial mass-like lesions, a rare and atypical presentation of the disease. Sarcoidosis typically involves the respiratory system, but its occurrence as endobronchial polyps mimicking malignancy is uncommon. The diagnosis was confirmed through bronchoscopy and biopsy, revealing non-caseating granulomas.

This study aimed to explore the atypical manifestations of Hansen's disease in a cohort of 15 biopsy-confirmed patients admitted to the Department of Dermatology, Venereology & Leprosy at a tertiary care center in Andhra Pradesh, India. There were 14 male patients and one female patient, with a mean age of 42.

Cavitary sarcoidosis is a rare form and represents non-caseating granulomatous diseases of the lungs exhibiting a narrow range of differential diagnoses. The peculiarity of this case lies in the difficulty of distinguishing atypical manifestations of pulmonary sarcoidosis, such as cystic lesions, from cavernous tuberculosis. Both possess similar clinical and radiological features.
