The spleen in ischaemic heart disease.

Nat Rev Cardiol

January 2025

Institute for Pathophysiology, West German Heart and Vascular Center, University of Duisburg-Essen, Essen, Germany.

Article Synopsis
  • Ischaemic heart disease results from coronary atherosclerosis, which is linked to systemic inflammation involving various immune cells released by the spleen.
  • Prolonged inflammation can lead to ischaemic heart failure, while the spleen's interaction with the nervous system can modulate immune responses and protect the heart from damage.
  • Splenectomy, which removes the spleen, increases mortality risk from ischaemic heart disease, highlighting the spleen's crucial role in immune responses and cardiovascular protection.

Septicemia After a Dog Bite: The Case of a Usual Suspect Transmitting an Unusual Organism.

Clin Pract Cases Emerg Med

August 2024

Inspira Health Network, Department of Emergency Medicine, Vineland, New Jersey.

Article Synopsis
  • This text discusses a rare case of antibiotic-resistant septicemia linked to canine bites in a 70-year-old man with a history of lymphoma, who showed severe symptoms and multiorgan failure after the incident.* -
  • The case highlights concerns about the transmission of infections from dogs to humans, particularly from oral microbiome bacteria that can be resistant to common antibiotics.* -
  • The conclusion emphasizes the importance of considering these infections in patients with severe sepsis after dog bites, along with the need for thorough medical histories and appropriate antibiotic selection to improve patient outcomes.*

Vago-splenic signal transduction of cardioprotection in humans.

Eur Heart J

September 2024

Institute for Pathophysiology, West German Heart and Vascular Centre, University of Essen Medical School, University of Duisburg-Essen, Hufelandstr. 55, 45147 Essen, Germany.

Background And Aims: The spleen serves as an important relay organ that releases cardioprotective factor(s) upon vagal activation during remote ischaemic conditioning (RIC) in rats and pigs. The translation of these findings to humans was attempted.

Methods: Remote ischaemic conditioning or electrical auricular tragus stimulation (ATS) were performed in 10 healthy young volunteers, 10 volunteers with splenectomy, and 20 matched controls.

Challenges in Diagnosing Post-Splenectomy Cytomegalovirus Mononucleosis.

Eur J Case Rep Intern Med

January 2024

Department of Internal Medicine, Trinity Health Oakland, Pontiac, USA.

Unlabelled: Cytomegalovirus (CMV) infection is often asymptomatic. However, in certain individuals, it can cause non-specific signs and symptoms that maybe hard to recognise. The condition may therefore be overlooked or misdiagnosed, leading to prolonged illness and serious sequelae.

Background: The spleen is the most commonly injured organ in abdominal trauma. Guidelines suggest non-operative management (NOM) is preferred over splenectomy for all haemodynamically stable patients, regardless of injury severity. The availability of splenic angioembolization has been shown to improve outcomes for high-grade splenic injuries by decreasing failure rates of NOM.
