A 60-year-old female patient was referred for further diagnosis and treatment of a posterior mediastinal lesion. The lesion was incidentally discovered in a CT scan of the chest and abdomen, which had been performed due to an increased CA 19-9 in routine blood tests. At the time point of the referral, the lesion had already been biopsied twice (CT guided and through EUS) but the histopathology was inconclusive.

High blood levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) have been shown in various malignancies. In lung cancer, the importance of NT-proBNP is not clear. In this study, we aimed to investigate the significance of the correlation of NT-proBNP levels in lung cancer with tumor stage, tumor diameter, histopathology, and specific sites of mediastinal metastasis: lymphadenopathy; pericardial, cardiac, major vessel, other mediastinal organ or lymphatic involvement/invasion.

Mediastinum, as site for malignant melanoma, is extremely rare, both as a primary or metastatic lesion. We present an extremely rare case of primary melanoma presenting as an anterior-superior mediastinal mass in a 65 years-old woman who went to the hospital for neurological symptoms due to unknown metastatic brain melanomas.
