Tentorial dural arteriovenous fistulas (DAVFs) are rare but highly dangerous vascular anomalies, constituting a small percentage of all intracranial DAVFs. Despite their infrequency, these lesions display aggressive characteristics, frequently leading to hemorrhage or neurological deficits due to their retrograde drainage into leptomeningeal veins, thus classifying them as Borden type III lesions. This case presents a middle-aged man who suffered cerebellar and subarachnoid hemorrhages resulting from a medial tentorial DAVF.

Background And Purpose: Stereotactic radiosurgery is a key treatment modality for cerebral AVMs, particularly for small lesions and those located in eloquent brain regions. Predicting obliteration remains challenging due to evolving treatment paradigms and complex AVM presentations. With digital subtraction angiography (DSA) being the gold standard for outcome evaluation, radiomic approaches offer potential for more objective and detailed analysis.

Background: The newly available ZAP-X stereotactic radiosurgical system is designed for the treatment of intracranial lesions, with several unique features that include a self-shielding, gyroscopic gantry, wheel collimation, non-orthogonal kV imaging, short source-axis distance, and low-energy megavoltage beam. Systematic characterization of its radiation as well as other properties is imperative to ensure its safe and effective clinical application.
Purpose: To accurately determine the radiation output of the ZAP-X with a special focus on the smaller diameter cones and an aim to provide useful recommendations on quantification of small field dosimetry.

Objective: Stereotactic radiosurgery (SRS) has been established as a safe and alternative treatment for dural arteriovenous fistulas (dAVFs). While embolization alone is the most commonly used modality for the treatment of dAVFs, the adjunctive use of embolization with SRS, with the growing use of SRS, has gained increasing interest in the past few years. However, the relative efficacy and safety of SRS combined with embolization versus SRS alone for dAVFs remains uncertain.
