Machine-learning-assisted Preoperative Prediction of Pediatric Appendicitis Severity.

J Pediatr Surg

January 2025

McGill University Faculty of Medicine and Health Sciences, Canada; Harvey E. Beardmore Division of Pediatric Surgery, The Montreal Children's Hospital, McGill University Health Centre, Montreal, Qc, Canada.

Purpose: This study evaluates the effectiveness of machine learning (ML) algorithms for improving the preoperative diagnosis of acute appendicitis in children, focusing on the accurate prediction of the severity of disease.

Methods: An anonymized clinical and operative dataset was retrieved from the medical records of children undergoing emergency appendectomy between 2014 and 2021. We developed an ML pipeline that pre-processed the dataset and developed algorithms to predict 5 appendicitis grades (1 - non-perforated, 2 - localized perforation, 3 - abscess, 4 - generalized peritonitis, and 5 - generalized peritonitis with abscess).

Objective: Identify rate and associated risk factors for dehiscence following gastrotomy for foreign material removal.

Methods: Medical records from 2 private practice emergency and referral hospitals were reviewed, and history, laboratory values, intraoperative findings, and outcomes were collected on 271 dogs and 31 cats (n = 302).

Results: Hospital A performed 222 procedures and Hospital B performed 80 procedures.

Intestinal obstruction is a rare but life-threatening incidence in pregnancy. Diagnosis can be challenging for clinicians as the symptoms might be approached as other common obstetric complications. Performing radiological and abdominal surgery are also areas of great concern in this field; since radiologic studies inevitably expose the fetus to radiation and the treatment options mostly involve surgery that is worrisome during gestation.

Aim: This study compared the short- and long-term efficacy of simple suture with omental patch repair (Graham patch) in open surgery versus laparoscopic omental patch repair (LOPR) in treating patients with Helicobacter pylori (H. pylori)-associated gastric ulcers with perforation, and analyzed the incidence of complications.

Methods: The clinical information of patients who had stomach perforation repair surgery in Deqing People's Hospital between January 2021 and January 2022 was retrospectively analyzed.

Cervical dilatation, uterine evacuation, and curettage (D&E&C) are common gynecological procedures for abortion, yet they carry risks of complications such as uterine perforation and intra-abdominal organ incarceration. Here, we report a rare case of a breastfeeding patient who had an embedded abdominal greater omentum in the anterior wall of the uterus and into the uterine cavity during D&E&C. We used combined hysteroscopic and laparoscopic treatment for this case and successfully removed the embedded greater omentum.
