An 18-year-old female patient presented with a 1-month history of low back pain, which had worsened and was accompanied by radiating pain in the right lower limb for half a month. She was admitted to our hospital with computed tomography and magnetic resonance imaging findings suggesting calcification of the L3/4 disc and a large intraspinal mass at the L2-4 level. The patient's symptoms did not improve with conservative treatment, and her muscle strength rapidly declined.

[Influencing fracture healing by specific osteoporosis medications].

Z Rheumatol

January 2025

Institut für Muskuloskelettale Medizin, LMU Klinikum, LMU München, München, Deutschland.

Background: Osteoporosis is a widespread disease defined by a reduction in bone mass and structure, thereby increasing the risk of fragility fractures. Treatment typically involves specific medications, which either inhibit bone resorption (antiresorptive) or stimulate bone formation (anabolic) and may potentially influence the healing of osteoporotic fractures. On the other hand, metabolic disorders, immune system dysfunctions or circulatory problems can impair fracture healing.

Apical periodontitis is an inflammatory disease caused by bacterial infection in the root canal that spreads to the apical periodontal tissues, resulting in bone resorption around the root apex as the disease progresses. Vascular endothelial growth factor (VEGF), a growth factor involved in angiogenesis, plays an important role in bone remodeling. We reported that caffeic acid phenethyl ester (CAPE), a bioactive substance of propolis, induces VEGF in odontoblast-like cells and dental pulp cells.

The ventricular system and subarachnoid space are filled with cerebrospinal fluid, which plays a key role in the nervous system. This fluid is produced by the choroid plexus, an organ rich in ion transporters that precisely control the transport of specific ions into the cerebrospinal fluid thanks to tight junctions between the plexus cells; these prevent the passage of substances other than the transporters, thus allowing for precise control of the fluid composition. Cerebrospinal fluid production is based on a network of interrelationships between specific ion flows enabled by the numerous transporters.

The active component in cannabis, cannabidiol (CBD), was first isolated from the hemp plant in 1940. Chronic pain, inflammation, migraines, depression, and anxiety have long been treated with CBD. The fundamental mechanisms of CBD's effects on periodontal inflammation have yet to be fully understood.
