Distinguishing between Parkinson's disease (PD) and essential tremor (ET) can be challenging sometimes. Although positron emission tomography can confirm PD diagnosis, its application is limited by high cost and exposure to radioactive isotopes. Patients with PD exhibit loss of the dorsal nigral hyperintensity on brain magnetic resonance imaging (MRI).

Background: Limited evidence exists regarding the meaningfulness of symptoms experienced in early Parkinson's disease (PD).
Objectives: To identify the most bothersome symptoms experienced by people with early PD, leveraging data from the Parkinson's Disease Patient Report of Problems (PD-PROP) questionnaire within the Fox Insight Study.
Methods: Individuals with a self-reported diagnosis of PD completed the PD-PROP questionnaire, reporting up to five most bothersome symptoms.

A 50-year-old woman with a 20-year history of gait instability presented with new-onset vertigo and oscillopsia. Examination revealed bilateral vestibular loss, cerebellar ataxia, sensory neuropathy, a "yes-yes" head tremor, nystagmus and a family history of a similar syndrome. Genetic testing for cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome (RFC1) was negative, but whole exome sequencing identified a novel mutation in the DNA methyltransferase 1 (DNMT1) gene, broadening the differential diagnosis for this phenotype.
View Article and Find Full Text PDFHistorically, screening for incidence of AD-related MCI or conversion from MCI to AD dementia has relied on cognitive, activities of daily living, and brain imaging measures. Limitations of this diagnostic approach include dependency on education and language, time-consuming and costly measures, and long-term monitoring. Emerging studies suggest that non-tremor motor dysfunction in dementias is known to be highly associated with AD biomarkers, with signs of cognitive decline visible in gait and hand movement at various stages of the illness.
