Background: Acute lung injury (ALI) is a disordered pulmonary disease characterized by acute respiratory insufficiency with tachypnea, cyanosis refractory to oxygen and diffuse alveolar infiltrates. Despite increased research into ALI, current clinical treatments lack effectiveness. Tetramethylpyrazine (TMP) has shown potential in ALI treatment, and understanding its effects on the pulmonary microenvironment and its underlying mechanisms is imperative.

Methods: This prospective, double-blind randomized controlled study included a total of 150 patients, scheduled for elective lower abdominal surgery under combined spinal-epidural (CSE) anaesthesia. The patients were randomised into three groups Group L (Lignocaine, n=50), Group DL (Dexamethasone, Lignocaine, n=50), and Group TL (Triamcinolone, Lignocaine, n=50).

The clinical use of dexamethasone (DXM) is associated with the development of non-alcoholic fatty liver disease (NAFLD). However, the mechanisms by which DXM-induced NAFLD is still incompletely known. Therefore, the current study aims to test the hypothesis that DXM-induced NAFLD is mediated by dysregulation of key genes involved in lipid metabolism and liraglutide (LG) can ameliorate these effects.
