Aims: The X-ray contrast medium Thorotrast, used worldwide between 1930 to 1950 predominantly for arteriography, consisted of a colloidal solution of thorium dioxide. The radioactive thorium-232 (half-life 1.4x10(10) years) is stored lifelong in the organs of the reticulo-endothelial system after intravascular injection, causing chronic exposure to alpha radiation.

A case of transitional cell carcinoma in the left kidney 25 years after urinary tract investigations, and squamous cell carcinoma in the right kidney 6 years later is described. Thorotrast was identified in the kidneys and in the reticulo-endothelial system post mortem and was presumably the contrast material used for the original retrograde pyelogram. When retained in the kidneys after retrograde pyelography Thorotrast produces a characteristic radiological and histological appearance and can be identified by the spectrum of radiation which it emits.

"Blockade" of the RES by thorotrast so lowered the tolerance of hemorrhagic shock in rabbits and dogs that a reversible degree of hemorrhagic shock became irreversible. This was true not only in normal rabbits, but in rabbits made resistant to hemorrhagic shock by producing resistance to endotoxins. Rabbits which had been pretreated with thorotrast and then subjected to hemorrhagic shock displayed at death the hemorrhagic lesions and the renal cortical necrosis characteristic of the Shwartzman reaction, in addition to the intramural hemorrhages in the gut which are characteristic of animals dying of hemorrhagic or of endotoxic shock.
