Detection of epileptogenic zones in people with epilepsy using optimized EEG-fMRI.

Epilepsy Behav

January 2025

Department of Neurology, Neurological Institute, Taipei Veterans General Hospital, Taipei, Taiwan; College of Medicine, National Yang Ming Chiao Tung University, Taipei, Taiwan. Electronic address:

Purpose: Concurrent electroencephalography (EEG) and functional magnetic resonance imaging (fMRI) have been used to assist in the presurgical localization of seizure foci in people with epilepsy. Our study aimed to examine the clinical feasibility of an optimized concurrent EEG-fMRI protocol.

Methods: The optimized protocol employed a fast-fMRI sequence (sampling rate = 10 Hz) with a spare arrangement, which allowed a time window of 1.

The aim of the study was to evaluate the concomitant psychiatric disorders of anxiety and depression in patients with epilepsy caused by low-grade brain tumors (LBTs). We retrospectively reviewed the clinical data of patients who underwent preoperative neuropsychological evaluations of anxiety and depression and subsequent epilepsy surgery for LBTs. The univariate and multivariate analyses were conducted to analyze the risk factors of the occurrence of anxiety and depression.

Nuclear Medicine Imaging in Epilepsy.

Radiographics

January 2025

From the Department of Radiology, University of Cincinnati Medical Center, 3188 Bellevue Ave, Cincinnati, OH 45219 (A.S., A.T.T., B.W.M., L.L.W., J.L.S.); and Department of Radiology, Cincinnati Children's Hospital and Medical Center, Cincinnati, OH (A.T.T.).

Approximately one-third of patients with focal epilepsy have medically refractory focal epilepsy (MRFE), which significantly impacts their quality of life. Once a seizure focus is identified and determined to be in the noneloquent cortex, it can be surgically resected with the goal of freedom from seizures and minimal neurocognitive deficit. During noninvasive (phase I) presurgical planning, functional (nuclear) imaging and structural imaging are complementary in the accurate localization of the epileptogenic zone (EZ).

. Accurate seizure prediction could prove critical for improving patient safety and quality of life in drug-resistant epilepsy. While deep learning-based approaches have shown promising performance using scalp electroencephalogram (EEG) signals, the incomplete understanding and variability of the preictal state imposes challenges in identifying the optimal preictal period (OPP) for labeling the EEG segments.

Article Synopsis
  • The study investigates the relationship between comorbid psychiatric disorders and pharmacoresistant temporal lobe epilepsy (TLE) associated with mesial temporal sclerosis (MTS), focusing on pre-surgical factors that could influence post-surgical psychiatric outcomes.
  • A retrospective analysis was conducted on 106 patients, with 51 having psychiatric disorders (PD) and 55 without, to identify predictors for post-surgical PD two years after cortico-amygdalohippocampectomy (CAH).
  • Results indicated that previous psychiatric treatment was linked to the development of PD post-surgery, with 35% of patients experiencing remission of PD and 14.5% developing new PD after CAH, underscoring the complexity of
