Infiltrating lipomas are a rare form of lipomas exhibiting unusual clinical behavior. We report a case of an adipocytic tumor of a 31-year-old male diagnosed with an infiltrating lipoma in the right submandibular region. It exhibits unusual clinical features such as invasion into surrounding structures, posing significant diagnostic challenges.

Background: Pelvic lipomas, particularly when massive in size, present unique surgical challenges due to their intricate anatomical location and proximity to vital structures. Complete resection of tumor is the basic principle. In this case, we demonstrated the complete resection of a huge pelvic lipoma by laparoscopy combined with a transsacral approach, which was rarely reported in the relevant literature.

Objective: Lipomatous soft tissue tumors (STT), ranging from benign lipomas to malignant liposarcomas, require accurate differentiation for timely treatment. Complementary to MRI, Contrast-enhanced ultrasound (CEUS) is emerging as a promising tool, providing insight into tumor microperfusion in real-time. This study aims to explore the potential of preoperative CEUS in differentiating benign lipomatous tumors from malignant liposarcoma subtypes.
