Eccrine acrospiromas, also known as hidradenomas, are rare benign tumors that develop from the eccrine sweat glands. Hidradenoma is a multilobular, nonencapsulated, well-circumscribed dermal nodule that may involve the epidermis and extend into the subcutaneous fat. The etiology and prevalence of nodular hidradenoma are not well defined, but it is noted that it can occur spontaneously or traumatically.

A young man in his 30s presented to us with multiple episodes of syncope and exertional dyspnoea for the last 2 weeks. He was diagnosed with squamous cell carcinoma of the lower one-third of the oesophagus in 2021 for which he was treated with neoadjuvant chemoradiotherapy, followed by McKeown oesophagectomy. At 2-year follow-up, he had developed a soft tissue swelling in the scalp, which was diagnosed as a tumour recurrence and radiotherapy was initiated.

A preferred option among many surgeons for treating large defects in the head and neck area is reconstruction using autologous tissue, particularly free tissue transfer with microvascular anastomosis. However, some defects cannot be resolved with conventional microvascular techniques or algorithmic approaches. In this case study, a 55-year-old female, who previously underwent bypass surgery for Moyamoya disease, presented with a large scalp defect following surgical necrosis.
