Endoprosthetic replacement of the pelvis is one of the most challenging types of limb-salvage surgery, with a high rate of complications. In an attempt to reduce this and build greater versatility into the reconstruction process, a new type of pelvic endoprosthesis was developed in 2003, based on the old McKee-Farrar prosthesis. This study reviews the outcomes in 27 patients who had an ice-cream cone pelvic prosthesis inserted at two different specialist bone tumour centres in the United Kingdom over the past six years.

Metal-on-metal total hip arthroplasty has the longest clinical history of any of the currently used articular couples. Long-term followup of what are now considered suboptimal designs (eg, McKee Farrar, Ring, Sivash) has produced a wealth of knowledge regarding the safety and efficacy of this articular combination. Retrieval analysis of metal-on-metal total hip arthroplasty articular couples has shown wear between 1 and 5 microns per year after initial wear-in, in comparison with 100 to 200 microns per year associated with metal-on-polyethylene wear.

Clinical and radiographic data for 15 McKee-Farrar hip replacements that had failed because of aseptic loosening (4 stem loosening, 9 cup loosening, and 2 loosening of both components) between 0.6 and 21 years (average, 8.3 years) were compared with 15 hips in which the McKee-Farrar total hip replacement has survived between 21 and 26 years.

Total hip arthroplasty was introduced early in Greece (1966-1967) and was initially performed in very small numbers. However, even after the difficult early period, statistics are low compared to other countries. An estimate brings the total number of operations during a 20-year period to 9000 with a rate in recent years of 1000 per year.
