Background: Pancreatic cancer involving the pancreas neck and body often invades the retroperitoneal vessels, making its radical resection challenging. Multimodal treatment strategies, including neoadjuvant therapy, surgery, and postoperative adjuvant therapy, are contributing to a paradigm shift in the treatment of pancreatic cancer. This strategy is also promising in the treatment of pancreatic neck-body cancer.

Introduction: Primary squamous cell carcinoma (SCC) is a rare type of pancreatic cancer with an extremely low incidence rate and a prognosis that is poorer than that of pancreatic ductal adenocarcinoma.
Presentation Of Case: We report a case of pure pancreatic SCC in an 80-year-old man. Based on the examination before surgical resection, we did not detect any SCC lesions that might have metastasized to the pancreas.

Purpose: There is no established surgical method for metastatic lesion to the pancreas. In the case of relatively small lesion, we often hesitate to select which surgical method, that is, wedge/partial resection or Whipple/distal pancreatectomy. Moreover, it is debatable whether lymph node dissection is necessary or not.
