Article Synopsis
  • - Aminophylline, a medication often used for severe asthma, can cause arrhythmias, but the exact reasons are not clear
  • - In research, aminophylline significantly inhibited the inward rectifier potassium current in both healthy pig heart cells and failing human heart cells at different concentrations
  • - The study found that aminophylline prolonged action potential duration in porcine heart cells, indicating its potential role in causing arrhythmias, suggesting further exploration of its effects on other ionic currents is necessary

The combination of aminophylline and salbutamol is frequently used in clinical practice in the treatment of obstructive lung diseases. While the side effects (including arrhythmias) of the individual bronchodilator drugs were well described previously, the side effects of combined treatment are almost unknown. We aimed to study the arrhythmogenic potential of combined aminophylline and salbutamol treatment in vitro.

Article Synopsis
  • Dipyridamole is commonly used for thallium-201 imaging to detect coronary arterial disease, and while it can cause mild noncardiac side effects like chest pain and headaches, these can usually be managed with aminophylline.
  • A case of a 45-year-old woman showed high-grade AV block and sinus arrest shortly after dipyridamole infusion, despite no signs of ischemia on the thallium scan and a normal baseline ECG.
  • The occurrence of AV block in this case raises concerns about bradyarrhythmias as a potential complication of dipyridamole, especially in those with existing conduction issues, and highlights the need for further investigation into the mechanisms behind such arrhythmias.

Background: Due to recurrent shortages of aminophylline, intravenous caffeine has emerged as a commonly used, safe and reliable method to treat adverse effects of vasodilator stress agents. We sought to evaluate the safety and effectiveness of buccal caffeine strips which are rapidly absorbed, inexpensive, readily available, and simplify caffeine administration.

Methods: Consecutive patients undergoing regadenoson stress SPECT MPI were assessed for the occurrence of symptoms during testing over an 11-week period at a single metropolitan hospital.

Bronchodilator aminophylline may induce atrial or less often ventricular arrhythmias. The mechanism of this proarrhythmic side effect has not been fully explained. Modifications of inward rectifier potassium (Kir) currents including I are known to play an important role in arrhythmogenesis; however, no data on the aminophylline effect on these currents have been published.
