Out of the 220 cases of goiters operated in the clinic 19 were partially or totally intra-thoracic, as follows: --totally intra-thoracic: 7 cases; --partially intra-thoracic: 12 cases, of which: mediastino-cervical: 2 cases; cervico-mediastinal: 10 cases. Five of the totally intra-thoracic goiters were in the anterior mediastinum, while two were in the posterior mediastinum, in a retro-oesophageal position. Three of the 19 intra-thoracic goiters were malignant. The symptomatology, the diagnosis and especially the therapeutical attitude are discussed.

Article Synopsis
  • * A study evaluating 36 patients who underwent TMIIL used a 3-cm linear-stapled side-to-side technique, with median operative time of 365 minutes and a 2.8% in-hospital mortality rate.
  • * Results indicate TMIIL with this method is generally safe, with low occurrence of anastomotic leaks (5.6%) and strictures, but some patients experienced complications like chylothorax and pulmonary issues.

Due to the difficulties in the intra-thoracic esophagogastric anastomosis creation, totally minimally invasive Ivor Lewis esophagectomy (MIE) did not encountered a large diffusion, preferring hybrid techniques or cervical anastomosis. Robot-assisted minimally invasive esophagectomy (RAMIE) has gained popularity due to an easy reproducibility of the open anastomotic technique. In this feasibility study, we described the RAMIE technique introduced in our Center, providing innovative details for a mechanical end-to-end anastomosis.

We describe a novel technique of totally robotic-assisted non-circumferential tracheal resection and running anastomosis with coverage of anastomosis with anterior mediastinal fat flap. A 71-year-old female presented with cough and CT scan revealed a mass at the intra-thoracic trachea. A complete robotic-assisted tracheal resection and anastomosis was performed.
