Nephrology nurses working in hemodialysis units face unique challenges managing multiple patients - an experience often contributing to higher levels of burnout and stress, and potentially lower job satisfaction and retention rates, exacerbating the existing nursing shortage in dialysis settings. Catatonia may manifest as an independent entity or as a feature of a neuropsychiatric or medical illness. While electroconvulsive therapy (ECT) is the gold standard treatment for catatonia, it is typically administered if the patient's symptoms fail to respond to benzodiazepines. We describe the case of a 22-year-old male with Budd Chiari induced cirrhosis and no prior psychiatric history, who presented with symptoms of psychosis and hepatic encephalopathy, was treated in the ICU for multi-factorial delirium, developed symptoms of catatonia that failed to respond to lorazepam, ultimately requiring ECT with a favorable response.

Moving away from our current trajectory towards 'hothouse Earth' will require knowledge of how Earth systems have varied throughout the Holocene, and whether and how far we have deviated from past ranges of variability. Such information can inform decisions about where change could be resisted, accepted or where adaptation is inevitable.

The surgical management of cancer patients wishing fertility preservation is multidisciplinary, involving surgeon, anesthetist, hematologist, and nursing and laboratory staff. Many oncology patients have a multitude of medical or surgical conditions that require careful planning of all therapy including surgical removal of reproductive material, either oocytes or ovarian tissue. The significant risks related to either transvaginal or abdominal surgery should be discussed and documented and the final decision to proceed must be balanced against the risks, including death.
