Introduction: Endothelin A (ETA) receptor activation is a driver of proteinuria, kidney inflammation, and fibrosis in IgA nephropathy (IgAN). Atrasentan, a selective ETA receptor antagonist, has potential to reduce proteinuria and preserve kidney function in IgAN. ALIGN (NCT04573478) is a phase 3, randomized, double-blind, placebo-controlled clinical trial of atrasentan in patients with IgAN at high risk of kidney function loss.

Direct oral anticoagulant (DOAC) use has significantly increased because major medical organizations endorse their role for conditions in which anticoagulation is indicated. Owing to important pharmacokinetic properties, the use of apixaban and rivaroxaban requires careful consideration in at-risk populations such as those with kidney disease. Both apixaban and rivaroxaban undergo some degree of renal elimination, and thus total drug exposure is increased in patients with renal insufficiency and/or those undergoing renal replacement therapy (RRT).

Pseudohaematuria Due to Mesalazine: A Case Report.

The symptom of macroscopic or 'visible' haematuria can cause significant patient distress, largely due to its' potential association with urinary tract malignancy, infection or glomerular disease. This lesson from practice describes the case of a 19-year-old female patient for whom the cause of red/brown urinary discolouration was found to relate to a reaction between renally excreted mesalazine and domestic bleach in the toilet bowel. Recognition of this phenomenon in patients taking mesalazine for inflammatory colitis is important to minimise patient distress and unnecessary investigation for a urinary tract cause.

Background: Rhabdomyolysis is frequently associated with acute kidney injury (AKI). Due to the nephrotoxic properties of myoglobin, its rapid removal is relevant. If kidney replacement therapy (KRT) is necessary for AKI, a procedure with effective myoglobin elimination should be preferred.
