Racecadotril in the management of diarrhea: an underestimated therapeutic option?

Therap Adv Gastroenterol

January 2025

Operative Unit of Clinical Pharmacology and Pharmacovigilance, Renato Dulbecco University Hospital, Catanzaro, Italy.

Acute infectious diarrhea (AID) represents an important clinical entity both regarding morbidity and mortality rates, even in industrialized countries, and it leads to one of the major public health burdens, among gastroenterological diseases, with significant healthcare costs. Oral rehydration solution is the cornerstone of the therapy, but despite its proven efficacy in avoiding dehydration, it is still underused as it does not reduce the duration of diarrhea; hence, it is perceived as ineffective by caregivers. In this narrative review, we collected literature regarding the use of racecadotril, deeply discussing its role in the treatment of AID in both adults and children.

Traumatic brain injury (TBI) is caused by an external mechanical force to the head, resulting in abnormal brain functioning and clinical manifestations. Antisecretory factor (AF16) is a potential therapeutic agent for TBI treatment due to its ability to inhibit fluid secretion and decrease inflammation, intracranial pressure, and interstitial fluid build-up, key hallmarks presented in TBI. Here, we investigated the effect of AF16 in an in vitro model of neuronal injury, as well as its impact on key components of the autophagy pathway and mitochondrial dynamics.

Article Synopsis
  • Somatostatin analogs (SSAs) are essential in managing hormonal symptoms and treating well-differentiated neuroendocrine tumors (NETs), established as first-line therapies through clinical trials like PROMID and CLARINET.
  • Recent updates highlight the use of emerging nonpeptide SSAs and innovative formulations in clinical settings to enhance treatment effectiveness.
  • Expert perspectives emphasize the pivotal role of SSAs in treatment strategies, while noting uncertainties in optimal dosing and potential benefits of new-generation SSAs in overcoming resistance.

Hospitalized patients on proton pump inhibitors for stress ulcer prophylaxis have a higher risk of Clostridioides difficile infection compared with those on histamine-2 receptor antagonists.

J Hosp Infect

December 2024

Institute of Clinical Pharmacy and Pharmaceutical Sciences, National Cheng Kung University, Tainan City, Taiwan; Department of Pharmacy, National Cheng Kung University Hospital, Tainan City, Taiwan. Electronic address:

Background: Previous studies on Clostridioides difficile infection (CDI) in proton pump inhibitor (PPI) users generally enrolled a heterogeneous population and did not include a control group of histamine H2 receptor antagonists (H2RAs) users or adjust for confounding variables, such as previous antibiotics. It is uncertain whether hospitalized patients using PPIs for stress ulcer prophylaxis (SUP) are at a higher risk of CDI compared with those using H2RAs. This study aimed to compare the association between CDI and the usage of antisecretory drugs (ASDs): PPIs and H2RAs, for SUP among hospitalized patients, and the impact of the duration of their use on CDI.
