Introduction: Leishmaniasis is a tropical parasitic disease caused by the protozoan Leishmania which remains a significant global health concern with diverse clinical manifestations. Transmitted through the bite of an infected sandfly, its progression depends on the interplay between the host immune response and the parasite. The disease outcome is linked to macrophage polarisation into M1 and M2 phenotypes.

We present a case of an 88-year-old man with symptoms consistent with a urinary tract infection, whose diagnostic workup uncovered a previously unrecognized motile flagellated protozoan. Molecular identification confirmed the organism as Dimastigella trypaniformis, a free-living kinetoplastid from the Rhynchomonadidae family. Known only from soil samples in Scotland and termite gut contents in Australia and Germany, Dimastigella trypaniformis has not been previously reported to infect vertebrate hosts.

Infection with the protozoan parasite causes human Chagas disease. Benznidazole (BNZ) and nifurtimox are the current drugs for the treatment; however, they induce severe adverse side effects in patients; therefore, there is a need to improve the treatment effectiveness and efficiency of these drugs for its safer use. : Glyburide, glipizide, and gliquidone, hypoglycemic drugs for diabetes treatment, were previously predicted to bind to dihydrofolate reductase-thymidylate synthase from by in silico docking analysis; they also showed antiproliferative effects against epimastigotes, the stage of the insect vector.
