The clinical records and microscopic sections from 32 patients were reviewed in an attempt to correlate histopathologic features with clinical behavior. Three general clinical groups were identified on the basis of recurrence rates and extent of papillomatosis. In those lesions with the highest frequency of recurrences, epithelial atypia tended to be most pronounced. Although some of the lesions were quite disturbing, there was no evidence of invasive carcinoma. It remains unknown to date whether these cytologic abnormalities represent a neoplastic alteration or possibly atypia on the basis of rapid, but benign, cellular turnover. Further studies at our center and other institutions are necessary to test the proposed classification which, if validated, would provide a means of predicting the course of the disease and efficacy of the chosen management in these difficult cases.

Background: Flat epithelial atypia (FEA), a rare breast proliferative lesion, is often diagnosed following core biopsy (CB) of mammographic microcalcifications. In the prospective multi-institution TBCRC 034 trial, we investigate the upgrade rate to ductal carcinoma in situ (DCIS) or invasive cancer following excision for patients diagnosed with FEA on CB.
Patients And Methods: Patients with a breast imaging reporting and data system (BI-RADS) ≤ 4 imaging abnormality and a concordant CB diagnosis of FEA were identified for excision.

Aims: Atypical ductal hyperplasia and flat epithelial atypia (FEA) have defined diagnostic criteria, yet there is variation in the interpretation of these criteria, particularly when the atypia is present in a background of columnar cell lesions (CCLs). This study focuses upon cases which are especially challenging or difficult to classify reproducibly according to existing criteria.
