IgD multiple myeloma (MM) is a rare subtype of myeloma, it affects less than 2% of patients with MM. To evaluate the clinical and prognostic attributes of serum free light chains (sFLCs) analysis, we examined 17 cases of IgD MM. From 1998 to 2012, we obtained 1250 monoclonal gammapathies including 590 multiple myeloma and 17 patients had IgD MM.

Objectives: Immunoglobulin D (IgD) multiple myeloma is an uncommon variant of the disease probably associated with poorer prognosis. However, data on IgD myeloma patients treated in the novel agent era are lacking.
Methods: To assess the frequency and the specific characteristics and evaluate the outcome of patients with IgD myeloma, we analyzed the database of the Greek Myeloma Study Group.

The study of IgD myeloma remains a challenging field. In terms of the initial detection of the IgD paraprotein, great care must be exercised in the interpretation of electrophoresis patterns and immunoglobulin profiles. Laboratory staff have a very important role to play in this, as it is likely that many IgD myeloma cases are uncovered following the involvement of laboratory staff.

The aim of this review was to compare the clinical characteristics and prognostic factors of Japanese and Western IgD myeloma patients, and also to introduce our new staging system, based on the analysis of Japanese patients. We reviewed 165 Japanese with IgD myeloma reported since the first Japanese case report in 1967 and compared these patients with Western IgD myeloma patients reported in the literature. Parameters evaluated were recorded at diagnosis.
