Currently, targeted biological therapy is an innovative and highly effective approach in the treatment of severe chronic rhinosinusitis with nasal polyps (CRSwNP) associated with T2 inflammation that is uncontrolled by standard treatment methods. The article presents the first experience of long-term observation of a patient who, one year after the start of targeted biological therapy with dupilumab, independently stopped the treatment due to complete relief of symptoms of CRSwNP, despite recommendations to continue therapy to maintain control. At the same time, one year after the refusal of biological therapy, the patient showed persistent clinical remission in relation to the symptoms of CRSwNP, confirmed by the results of videoendoscopic examination of the nasal cavity and computed tomography of the sinuses.

Background: Although rare, vaccine-induced thrombotic thrombocytopenia (VITT) following adenoviral vector COVID-19 vaccination is a concerning and often severe adverse effect of vaccination. The generation of high antiplatelet factor 4 antibody titers promotes the formation of immune complexes capable of activating platelets and neutrophils through FcγRIIa.

Objectives: Given that platelet-leukocyte aggregate formation and inflammasome activation are common features of thromboinflammatory diseases, we aimed to evaluate if these are also features of VITT.

Recent studies underscore the anti-inflammatory role of SIRT1; however, its levels during inflammatory states remain ambiguous. We synthesized relevant studies up to 20 March 2024 to evaluate the relationship between SIRT1 and inflammation, using data from three major databases. Employing a random-effects model, we analyzed both cross-sectional and longitudinal studies, calculating weighted mean differences (WMDs) for pooled effect sizes.
