Posterior shoulder approach.

A posterior shoulder surgical approach combines the subperiosteal detachment of the deltoid from the scapular spine, described by McWhorter, with a lateral extension to include the lateral deltoid splitting of Codman. The glenohumeral joint is superiorly approached between the supraspinatus and infraspinatus tendons, or inferiorly between the infraspinatus and teres minor tendons. This approach was used in 42 shoulders with rotator cuff tears or posterior instability without complications of infection, failure of deltoid healing, or compromise of suprascapular or axillary nerves. This combined surgical approach provides excellent exposure of the superior and posterior shoulder, which is difficult to obtain by anterior or superior approaches. This approach may be used for repair of rotator cuff tears, posterior instability, and displaced posterior glenoid fractures.

Background: The purpose of this study was to assess impingement-free internal rotation (IR) in a virtual reverse shoulder arthroplasty simulation using a Statistical Shape Model based on scapula size.

Methods: A database of over 10,000 scapulae utilized for preoperative planning for shoulder arthroplasty was analyzed with a Statistical Shape Model to obtain 5 scapula sizes including the mean and 2 standard deviations. For each scapula model, one glenosphere size (33-42 mm) was selected as the best fit based on consensus among 3 shoulder surgeons.

Introduction: Reverse shoulder arthroplasty (RSA) and anatomic total shoulder arthroplasty (TSA) are well-known methods of treating glenohumeral arthritis, which often leads to posterior wear of the glenoid. This study compared minimum two-year outcome measures in patients treated with RSA and TSA for Walch B2 and B3 glenoids.
