Peri-Intubation Cardiac Arrest in a Patient With Hypertensive Cardiac Tamponade.

Cardiac tamponade is widely known to be associated with life-threatening hypotension and rarely with hypertension. We present the case of a 53-year-old woman with hypertension and echocardiographic features of tamponade who had a cardiac arrest on intubation. The paradoxical hypertension is thought to be due to tachycardia and increased peripheral vascular resistance resulting from the compensatory sympathetic surge following impaired cardiac filling caused by the effusion.

We report a case of an unusual polypharmacy overdose including warfarin in a patient with a metallic heart valve, complicated by the history that he had ceased taking anticoagulation in the preceding few months, that placed him in an initial prothrombotic phase during his presentation. Our case highlights the importance of elucidating all relevant clinical details in a polypharmacy overdose, as clearly and rapidly as feasible, to enable appropriate management.

Unlabelled: Right-to-left shunt may be a cause of paradoxical embolization (and cryptogenic cerebral ischemic events). Patent foramen ovale and pulmonary arterio-venous malformations are the most common cause of paradoxical embolization; however, some rare cardiovascular lesions may cause a right-to-left shunt with consequent paradoxical embolization. We describe a case of a young child with superior sinus venosus atrial septal defect who underwent surgical closure and developed a cryptogenic cerebral ischemic attack due to a rare cause of paradoxical embolization, during a long-term follow-up.
