In this opinion piece, we examine the pivotal role that uncertainty quantification (UQ) plays in informing clinical decision-making processes. We explore challenges associated with healthcare data and the potential barriers to the widespread adoption of UQ methodologies. In doing so, we highlight how these techniques can improve the precision and reliability of medical evaluations. We delve into the crucial role of understanding and managing the uncertainties present in clinical data (such as measurement error), diagnostic tools and treatment outcomes. We discuss how such uncertainties can impact decision-making in healthcare and emphasize the importance of systematically analysing them. Our goal is to demonstrate how effectively addressing and decoding uncertainties can significantly enhance the accuracy and robustness of clinical decisions, ultimately leading to better patient outcomes and more informed healthcare practices.This article is part of the theme issue 'Uncertainty quantification for healthcare and biological systems (Part 1)'.

Background: This article discusses the ethical issues surrounding the integration of long-acting injectable antiretroviral therapy (LA-ART) in the programmatic management of human immunodeficiency virus (HIV). As the medical landscape evolves, implementing LA-ART introduces many ethical issues that should be considered for the success of scale-up in diverse settings.
Methods: This article examines key issues such as bioethical concerns around the rollout of LA-ART, including regulatory requirements, a person's autonomy, informed consent, privacy and confidentiality; the societal implications of providing LA-ART, including the impact on stigma and discrimination; ethics around who receives LA-ART, financial accessibility, equitable access, inclusive decision-making and cultural sensitivity; and the ethics of providing an expensive intervention, including cost-effectiveness, supply chain sustainability and resource allocation.

Hypertension, a globally prevalent condition, is closely associated with T cell-mediated inflammatory responses. Studies have shown that T cells, by secreting pro-inflammatory cytokines such as interferon-gamma (IFN-γ), Interleukin-17 (IL-17), and Tumor necrosis factor-alpha (TNF-α), directly lead to vascular dysfunction and elevated blood pressure. The activation of Th1 and Th17 cell subsets, along with the dysfunction of regulatory T cells (Tregs), is a critical mechanism in the onset and progression of hypertension.

Methods: This comprehensive review addresses the significant advances made in oocyte in vitro maturation with the biphasic capacitation (CAPA)-IVM strategy applied to small ovarian antral follicles in humans over the last 10 years. CAPA-IVM consists of a prematuration phase wherein immature oocytes are temporarily meiotically arrested to gain competence before undergoing meiotic resumption.
