This case report describes a two-stage surgical approach for managing a traumatic defect of the left trapezium bone. A 51-year-old male presented following a high-impact injury caused by a heavy iron object striking his left wrist. The patient reported severe wrist pain and restricted thumb motion. The patient sustained a severe wrist injury resulting in extensive bone loss. Initial treatment involved debridement and bone cement filling, followed by a second-stage implantation of a patient-specific 3D-printed titanium prosthesis. At 24-month follow-up, the patient demonstrated excellent thumb functionality and significant clinical improvement. This case illustrates the successful use of a 3D-printed trapezium prosthesis to treat a complex comminuted fracture with bone defect. This method offers a promising alternative for managing severe carpal fractures.

Arthrosis of the trapeziometacarpal joint of the thumb is a predictable sequelae of ligament laxity. A new technique of tendon interposition arthroplasty with ligament reconstruction using the flexor carpi radialis tendon for a painful arthritic trapeziometacarpal joint of the thumb is described. Thumb motion is a key outcome metric for assessing disease progression or treatment efficacy. A literature review found nearly 25 % of recent papers incorrectly described their motion measurements as those of the first carpometacarpal (CMC) joint, when in fact their technology was only capable of measuring thumb motion. The aim of this manuscript is to clarify the importance of the accurate terminology and to rigorously examine the potential error by comparing thumb motion and CMC joint motion.

Difference between the thumb motions in carpal tunnel syndrome and trapeziometacarpal osteoarthritis.

Background: This study quantified thumb movement in patients with trapeziometacarpal osteoarthritis and carpal tunnel syndrome and examined the characteristics of each disorder using three-dimensional motion analysis.

Methods: Twenty-three hands of 23 patients with trapeziometacarpal osteoarthritis, 88 hands of 73 patients with idiopathic carpal tunnel syndrome, and 29 hands of 20 healthy volunteers were included in this study; three-dimensional thumb kinematics were measured during the circumduction task using a retroreflective surface-based marker method.

Findings: Trajectory of thumb tip reduced in both patient groups compared to healthy controls, with carpal tunnel syndrome exhibiting the flattening shape.
