Introduction: Mpox is a viral disease that primarily affects individuals living in endemic regions. The 2022 outbreak notably impacted HIV-positive individuals, who were disproportionately affected. This report describes the first confirmed case of Mpox in Burundi, involving an HIV-positive patient with advanced disease. The case was confirmed by the national laboratory during the early stages of the 2024 outbreak.
Case Presentation: A 50-year-old woman presented with pustular and necrotic skin lesions, vulval ulcers, and systemic symptoms. She was diagnosed with HIV upon initial evaluation and, despite an initial misdiagnosis, was ultimately confirmed to have a co-infection of Mpox. Complications included secondary bacterial infection, severe pain, and prolonged healing.
Conclusion: HIV coinfection can exacerbate Mpox, leading to severe clinical presentations. Therefore, early recognition and screening for HIV in Mpox patients are essential.

In such cases, effective patient management is dependent on both rapid identification of disease and the provision of necessary medical care while adhering to suitable infection prevention and control measures to reduce the potential for onwards transmission. The UK has operated a defined system for managing patients with high-consequence infectious diseases (HCIDs) since the 1970s, when protocols were first implemented following the first descriptions of several viral haemorrhagic fever diseases, including Marburg virus disease, Lassa fever and Ebola virus disease (EVD).

To address the underreporting of mpox cases in endemic regions, a regional surveillance network, known as the Mpox Threat Reduction Network (MPX-TRN), was established between five neighboring countries in Central and West Africa in 2022. One direct outcome of the MPX-TRN has been the strengthening of regional mpox surveillance. This consortium has facilited open communication channels, detection of cross-border mpox cases, and improvements of the detection and diagnosis of mpox in Central Africa and worldwide.

We report a case of mpox in a patient with a signal transducer and activator of transcription 1 gain-of-function mutation. Despite initial improvement with intravenous immune globulin and tecovirimat, severe symptoms developed and the patient died. This underscores the need for immune system optimization and effective virucidal treatments for mpox.
