Ischemic steal syndrome (ISS) occurs in hemodialysis patients. Hence, it rarely becomes gangrene or disrupts hand function. We reported a patient with an ISS and radial atherosclerosis that presented with infected ulcer. The patient was treated with arterialized venous free flap from the contralateral forearm. Dialysis access can be used as early as 10 days after the flap. The report also describes the long-term functional follow-up after reconstruction. Although there was superficial flap necrosis, this non-physiologic flap can preserve the patient's hand function.

Sex differences in the risk factors, diagnosis, treatment, and outcomes of patients with cardiovascular disease have been well described; however, the bulk of the literature has focused on heart disease in women. Data on sex differences in peripheral vascular disease are ill defined, and there is a need to report and understand those sex-related differences to mitigate adverse outcomes related to those disparities. Although peripheral vascular disease is a highly diverse group of disorders affecting the arteries, veins, and lymphatics, this scientific statement focuses on disorders affecting the peripheral arteries to include the aorta and its branch vessels.

Endovascular embolization is a minimally-invasive technique whereby blood vessels supplying pathological structures are selectively occluded with various embolic agents. In many scenarios, it is desirable for the embolic to distally penetrate to the level of the microvasculature, which maximizes devascularization. Existing agents exhibit inconsistent distal penetration and have other limitations including tendency for proximal reflux, patient pain during infusion, lack of fluoroscopic radiopacity, potential for catheter adhesion, susceptibility to recanalization, and other usability challenges.
