A Race Against Time: Rapidly Progressive Methicillin Resistant Pansinusitis with Vision Loss in an Immunocompetent Child.

Published: February 2025

Acute sinusitis is a relatively common condition in pediatric patients. In most cases, it resolves completely with prompt medical management, but its rapid progression to severe orbital complications leading to restriction of extra-ocular movements and negative perception of light in the affected eye is rare and pose diagnostic and management challenges. The aggressive nature of acute rhinosinusitis requires urgent surgical intervention coupled with prompt medical management to prevent life threatening complications and outcomes. We present a case of a 15-year-old male with no history of trauma or immunosuppression, initially presented with symptoms of upper respiratory tract infection but rapidly developed left orbital cellulitis. Timely diagnosis and multidisciplinary management, including urgent surgical intervention with endoscopic sinus surgery and orbital decompression led to successful resolution of symptoms and prevention of further complications. Nonetheless, our race against time could not reverse the loss of vision.
