Syndesmotic injuries of the ankle are critical for maintaining joint stability and function, especially during weight-bearing activities. While traditional management involves rigid fixation with trans-syndesmotic screws, limitations such as restricted micromovement, potential screw breakage, and the need for secondary procedures have spurred the adoption of dynamic fixation methods like the TightRope system (Arthrex, Naples, Florida, US). This report presents a case of a 54-year-old female with complications related to TightRope fixation following open reduction and internal fixation (ORIF) of a bimalleolar fracture. The case highlights the clinical course, surgical management, and outcomes, providing valuable insights into the limitations and potential complications of this technique.

Connective tissue disorders are known to cause cardiac and vascular complications. We present the case of a 37-year-old female patient with a rare Ehlers Danlos/Osteogenesis Imperfecta Overlap Syndrome, referred to cardiac surgery with aortic valve regurgitation, who underwent a successful Biological Surgical Aortic Valve Replacement (SAVR). A multidisciplinary, patient-centered, heart-team approach is essential in managing patients with rare genetic disorders to optimize postoperative outcomes.

Background: Olfactory neuroblastoma (ONB) is a rare malignancy that develops in the nasal cavity, with appendicular skeletal metastases being exceptionally uncommon. We report the case of a middle-aged male with ONB, presenting with an unusual biopsy-confirmed solitary metastasis to the left proximal femur.
Case Presentation: A 33-year-old male was previously diagnosed with olfactory neuroblastoma in the skull base after initial clinical presentation with progressive left-sided nasal obstruction for six months.

Portal vein thrombosis (PVT) is a rare but potentially severe condition that is typically associated with underlying haematological disorders, genetic mutations, or liver diseases such as cirrhosis. However, PVT resulting from acute cholecystitis is an exceedingly uncommon occurrence with few documented cases. This report describes the case of a 44-year-old man who presented with acute right upper quadrant pain and was diagnosed with acute cholecystitis complicated by left-sided PVT, which was managed with anticoagulants and laparoscopic cholecystectomy.
