In this case report, a 69-year-old man with a 25-year history of type 2 diabetes mellitus was admitted with severe symptoms like flank pain radiating to the groin, high-grade fever, hematuria, and respiratory symptoms, including shortness of breath and a productive cough. After a complete evaluation, he was diagnosed with emphysematous pyelonephritis (EPN), and to treat this condition, the patient underwent a surgery called percutaneous nephrolithotomy (PCNL), which is used to remove kidney calculi. EPN is a condition characterized by severe infection of the kidney tissue.

Melanoma is a clinically and genetically heterogeneous malignancy that can recur long after initial treatment. We report the case of a 70-year-old man who presented with metastatic cutaneous melanoma 36 years after the initial diagnosis. Initially diagnosed with nodular melanoma on the left heel at age 34 years, the patient experienced metastatic spread to the left groin lymph nodes by age 36 years, followed by various systemic therapies, including interferon and cytokine therapy, and subsequent observation, over 15 years.

Introduction: Lyme borreliosis has been associated with lymphoma, particularly cutaneous lymphomas. The literature is conflicted regarding the effect of antibiotic therapy in cutaneous marginal zone lymphomas (MZLs) in individuals with Lyme borreliosis. We present a patient diagnosed with Lyme neuroborreliosis (LNB) and disseminated MZL.
