Pulmonary vasodilators increased the systemic-to-pulmonary collateral flow in two single-ventricle cases, causing haemodynamic decline. Discontinuation reversed the collateral flow and improved heart failure, highlighting the need for careful monitoring. Serial cardiac MRI, as a non-invasive tool, may be essential to detect changes in collateral flow and guide vasodilator use for optimal management in this population.

Collateral blood vessels are unique, naturally occurring endogenous bypass vessels that provide alternative pathways for oxygen delivery in obstructive arterial conditions and diseases. Surprisingly however, the capacity of the collateral circulation to provide protection varies greatly among individuals, resulting in a significant fraction having poor collateral circulation in their tissues. We recently reviewed evidence that the presence of naturally-occurring polymorphisms in genes that determine the number and diameter of collaterals that form during development (ie, genetic background), is a major contributor to this variation.

Diabetes-induced premature senescence of endothelial cells (EC) has been proposed as a mechanism leading to impaired ischemia-driven angiogenesis. Background And Objectives: It is unknown whether acute CT perfusion (CTP) core imaging may underestimate the follow-up infarct. We hypothesize that infarct underestimation occurs especially in late-presenting patients and that underestimated infarct can partially be detected on baseline noncontrast CT (NCCT).

Computed tomography for major aortopulmonary collateral arteries: a roadmap to unifocalized pulmonary arterial flow.

In conjunction with catheter angiography, high-resolution, isotropic, three-dimensional computed tomography mapping of major aortopulmonary collateral arteries is useful for delineating complex thoracic arterial anatomy and has been increasingly utilized in patients with pulmonary atresia and ventricular septal defect. The presence, confluence, and size of the central (mediastinal native) pulmonary artery; the presence and course of a patent ductus arteriosus; the number of collateral arteries; the number of lung segments supplied by each collateral artery; significant distal pulmonary artery stenoses; and isolated or dual blood supply in each lung segment can be evaluated using computed tomography mapping. The detailed mapping can substantially reduce procedural time for catheter angiography, which delivers a higher radiation dose than computed tomography does, and is useful for surgical planning.
