Background: Acute rheumatic fever is a systemic inflammatory condition caused by beta-hemolytic streptococcal infection. Its prevalence in high-income countries has fallen significantly as a result of widespread antibiotic use and improved living conditions.
Case Summary: We report the case of an 18-year-old, otherwise healthy, Irish woman who presented to an Irish hospital with fever and general malaise on a background of recurrent pharyngitis. Her condition rapidly deteriorated in the hospital and she developed septic shock requiring intensive care admission for organ support. Investigations confirmed pancarditis secondary to acute rheumatic fever, which was ultimately treated with antibiotics and corticosteroids.
Discussion: This case highlights that acute rheumatic fever can present with multiple nonspecific symptoms, in unexpected populations, including in the developed world. Early diagnosis and treatment initiation of antimicrobial therapy and corticosteroids is imperative for optimal overall outcomes in rheumatic heart disease.

Objective: To investigate the risk factors for severe acute COVID-19 outcomes in the Omicron era among individuals with systemic autoimmune rheumatic diseases (SARDs).
Methods: We identified patients with confirmed SARDs and COVID-19 (positive PCR and/or antigen test) from 1 September 2022 to 15 March 2024 in the Mass General Brigham healthcare system. We estimated the associations of baseline characteristics with the odds of hospitalisation due to COVID-19 infection, verified by medical record review, using multivariable logistic regression.

Intestinal perforation, a rare complication of interleukin (IL)-6 therapy for immune-mediated diseases (mainly rheumatoid arthritis), typically manifests in the lower gastrointestinal tract, often in association with prior history of diverticulitis. Patients may present with acute abdominal pain and suspicion for this complication should remain high even in the absence of elevated C-reactive protein. We describe a 69-year-old female patient with a history of resistant seropositive palindromic rheumatism treated with sarilumab who developed a nontraumatic terminal ileal perforation.
