Background: Superior vena cava (SVC) aneurysms are rare vascular anomalies, with <80 cases reported worldwide. Large aneurysms carry a risk of rupture and thrombosis, eventually necessitating surgical intervention.

Case Summary: We present the case of a 68-year-old male with a progressively enlarging SVC aneurysm and a history of paroxysmal atrial fibrillation. The patient underwent successful caval resection and replacement with a 20 mm expanded polytetrafluoroethylene (ePTFE) graft, accompanied by left atrial appendage exclusion and pulmonary vein isolation. The recovery from surgery was uneventful, and postoperative imaging confirmed excellent graft patency with no signs of thrombosis or recurrence at the three-month follow-up.

Discussion: This case highlights the successful management of a large SVC aneurysm through complete resection and prosthetic replacement with a 20 mm ePTFE graft, with the first documented use of this technique in an adult patient. Caval resection and graft replacement ensure complete excision of affected tissue, minimizing the risk of recurrence after surgery for complex SVC aneurysms.

Background: Lymphomas, which originate from the haematopoietic system, are seldom found in the heart due to the absence of a lymphoid system. Primary cardiac lymphoma is quite rare. Cardiac lymphomas can present with dyspnoea, heart failure, pericardial effusion, and arrhythmia.

Background: Pedal acceleration time (PAT) is a novel indicator of peripheral arterial disease in the lower limbs. Elevated PAT values are associated with worse limb ischemia. Arterial stiffness indexes are another class of indicators recently studied in patients with chronic limb-threatening ischemia (CLTI).

Heart rate recovery (HRR) is a critical indicator of cardiovascular fitness and autonomic nervous system function, reflecting the balance between sympathetic and parasympathetic activity. Slower HRR is often linked to cardiovascular and metabolic disorders, highlighting its potential for identifying high-risk individuals. In this study, we developed a feature engineering approach integrated to wearable device data to classify individuals into high-risk (slower HRR) and low-risk (faster HRR) groups.

Home Monitoring of High-Risk Individuals Receiving Opioids Post Orthopedic Surgery.

Respir Care

March 2025

Postoperative patients receiving opioids are at risk for morbidity and mortality caused by opioid-induced respiratory depression (OIRD). Guidelines advocate electronic monitoring for OIRD during postoperative hospitalization, but the utility of home monitoring following ambulatory surgery has not been assessed. We describe the utilization of capnography and pulse oximetry in an ambulatory orthopedic cohort to determine subject/home caregiver acceptance and utility of continuous monitors at home.

Individuals with comorbid rapid eye movement (REM) sleep behaviour disorder (RBD) and neurotrauma (NT; defined by traumatic brain injury and post-traumatic stress disorder) have an earlier age of RBD symptom onset, increased RBD-related symptom severity and more neurological features indicative of prodromal synucleinopathy compared to RBD only. An early sign of neurodegenerative condition is autonomic dysfunction, which we sought to evaluate by examining heart rate variability during sleep. Participants with overnight polysomnography were recruited from the Veterans Affairs Portland Health Care System.
