Giant inguinoscrotal (GIS) hernias are rarely present in clinical settings and its treatment with a minimally invasive approach is often considered a challenge. In recent years, robotic surgical techniques have gained prominence in the medical field, offering advantages over traditional laparoscopic methods. The Hugo robotic system introduces a fresh paradigm by employing independent robotic arms, enhancing manoeuverability in diverse surgical contexts. The aim of this video is to evaluate the efficacy of robotic TAPP (robotic transabdominal preperitoneal) in order to treat a GIS hernia.

This article examines the emergency surgical management of giant bilateral inguinoscrotal hernias through a representative case report. In this case, surgical intervention was required due to an associated cecal volvulus. While acute complications such as bowel strangulation are rare in hernias of this size, delayed presentation and the lack of timely surgical consultation permit significant inguinoscrotal hernia growth.

The surgical repair of giant inguinal hernias with loss of domain, defined as the relocation of the majority of the intestine into the hernia sac, poses a significant challenge. In the majority of cases, a combination of different surgical techniques with the placement of multiple meshes is necessary to achieve reduction of such complex hernias. The reduction of chronic giant hernias can increase the risk of abdominal compartment syndrome or cardiopulmonary complications.

Purpose: Management of giant inguinoscrotal hernia (GIH) is still a challenging procedure associated with a higher risk of intraabdominal hypertension and abdominal compartment syndrome as a life-threatening condition. The aim of the study was to present our management strategy for GIH.
Methods: This is a retrospective review of a case series including 24 consecutive patients with 25 GIH who underwent reconstructive surgery from January 2006 to June 2023, at the University Clinic for Digestive Surgery and Hernia Center Zuvela.
