Human immunodeficiency virus (HIV) infection is a strong risk factor for diffuse large B-cell lymphoma (DLBL) and tuberculosis. Both DLBCL and tuberculosis can have remarkably similar clinical presentations, proving to be a diagnostic and therapeutic challenge. We report the only known case of an HIV-infected individual who presented simultaneously with relapsed DLBCL in the form of spinal cord involvement and tuberculosis of the mesenteric lymph nodes. This case highlights the possibility of multiple co-existing diagnoses in HIV, and the need for a low threshold to obtain confirmation via biopsy. The interferon-gamma release assay (IGRA) has low sensitivity in detecting tuberculosis in patients with HIV. Adherence to antiretroviral therapy (ART) is crucial in achieving and maintaining remission in DLBCL.

Diffuse large B-cell lymphoma (DLBCL) and chronic lymphocytic leukemia (CLL) are subtypes of non-Hogkin lymphoma (NHL) that are generally distinct form one cases, but the transformation of one of these diseases into the other is possible. Some patients with CLL, for instance, have the potential to develop Richter transformation such that they are diagnosed with a rare, invasive DLBCL subtype. In this study, bioinformatics analyses of these two NHL subtypes were conducted, identifying key patterns of gene expression and then experimentally validating the results.

Lymphoma, a malignant tumor derived from lymphocytes and lymphoid tissues, presents with complex and heterogeneous clinical manifestations, requiring accurate patient classification for appropriate treatment. While invasive pathological examination of lymph nodes or lymphoid tissue remains the gold standard for lymphoma diagnosis, its utility is limited in cases of deep-seated tumors such as intraperitoneal and central nervous system lymphomas. In addition, biopsy procedures carry an inherent risk of complications.
