We report a case of a 76-year-old female with stage 4 chronic kidney disease (CKD) who experienced multiple adverse events following the initiation of colchicine for a gout flare. Diarrhea led to further complications, including acute renal failure, hypercalcemia, and bradycardia. The patient additionally developed the less common toxicities of myelosuppression and presumed myopathy, resulting in prolonged hospitalization with persistent functional impairment at the time of discharge. This report highlights the importance of exercising caution when prescribing colchicine to patients with severe CKD. It discusses alternate ways to manage acute gout as well as strategies to minimize the risk of adverse events when using colchicine in this population.

The gut-kidney axis is the bidirectional relationship between the gut microbiota and the kidney function. Chronic inflammatory responses can impair kidney function and probiotics and postbiotics agents can have positive effects on gut health and kidney function by modulating inflammation through affecting autophagy signaling pathway. Background: There is an international epidemic of chronic kidney disease of unknown cause (CKDu) in agricultural working populations. Particulate air pollution is a likely contributing factor in populations at risk for CKDu, but there is little personal breathing zone data for these workers.
Methods: We collected 1 to 3 personal breathing zone particulate matter <5 microns (PM5) gravimetric measurements in 143 male sugarcane harvesters over 2 seasons and concurrent ambient samples using personal sampling pumps and cyclone inlets as a sampling train.
