Ischemic stroke is a common disease that can stem from multiple different causes, some of which are uncommon. A 43 year old woman with a history of systemic lupus erythematosus and antiphospholipid syndrome visited the emergency department due to dysarthria and was diagnosed with ischemic stroke. Transesophageal echocardiogram showed a mass on the mitral valve. After further workup, she was diagnosed with Libman-Sacks endocarditis, which is caused by endothelium damage along with hypercoagulability, and can form due to systemic lupus erythematosus or antiphospholipid syndrome. Libman-Sacks endocarditis is a nonbacterial thrombotic endocarditis and thromboembolism is rare. It is treated with anticoagulation. It is also important to treat the underlying cause of the endocarditis.

Purpose: To compare the periprocedural and 5-year outcomes between patients with very severe stenosis (80%-99% stenosis) and those with carotid near occlusion (CNO) with full collapse.
Materials And Methods: Data from patients with very severe stenosis and CNO with full collapse who underwent carotid artery stenting (CAS) at our center were retrospectively analyzed from January 2018 to December 2021. Unmatched and propensity score-matched (PSM) comparisons of periprocedural complications and 5-year follow-up rates for in-stent restenosis, ipsilateral ischemic stroke, myocardial infarction (MI), and mortality were conducted between the two groups.

Ferroptosis contributes to neuronal destruction after ischemic stroke which may be improved by inhibiting BTB domain and CNC homologue 1 (BACH1), a recently recognized ferroptosis facilitator. Axon guidance molecule netrin-1 (Ntn1) functions in neuroprotection against ischemic insult by engaging into its receptor of uncoordinated-5 homolog B (UNC5b) via adenosine 5'-monophosphate-activated protein kinase (AMPK), which potentially binds to BACH1. Whether Ntn1/UNC5b regulates post-stroke ferroptosis through AMPK-BACH1 pathway remains unclear.
