Sudden unexpected death in epilepsy (SUDEP) is a critical concern for individuals suffering from epilepsy, with respiratory dysfunction playing a significant role in its pathology. Fatal seizures are often characterized by central apnea and hypercapnia (elevated CO levels), indicating a failure in ventilatory control. Research has shown that both human epilepsy patients and animal models exhibit a reduced hypercapnic ventilatory response in the interictal (non-seizure) period, suggesting an impaired ability to regulate breathing in response to high CO levels. This review examines the role of central chemoreceptors-specifically the retrotrapezoid nucleus, raphe nuclei, nucleus tractus solitarius, locus coeruleus, and hypothalamus in this pathology. These structures are critical for sensing CO and maintaining respiratory homeostasis. Emerging evidence also implicates neuropeptidergic pathways within these chemoreceptive regions in SUDEP. Neuropeptides like galanin, pituitary adenylate cyclase-activating peptide (PACAP), orexin, somatostatin, and bombesin-like peptides may modulate chemosensitivity and respiratory function, potentially exacerbating respiratory failure during seizures. Understanding the mechanisms linking central chemoreception, respiratory control, and neuropeptidergic signaling is essential to developing targeted interventions to reduce the risk of SUDEP in epilepsy patients.

During the COVID-19 outbreak, N-nitrosodimethylamine (NDMA) and N-nitrosomorpholine (NMOR) and their specific precursors (N,N-dimethylformamide [DMF] for NDMA and morpholine [MOR] for NMOR) were widely detected in sewerage systems of an urban area, in which chlorine-containing disinfectants were discharged with effluent of hospitals, etc. However, little is known about the effect of chlorine influx on formation and distribution of NDMA and NMOR in sewer systems in a sudden major public health event. We investigated the spatiotemporal patterns of NDMA, NMOR, DMF and MOR in influents of sewage treatment plants (STPs), as well as its upstream sewer sites during the COVID-19 pandemic.

Melatonin has been shown to improve sleep quality in Parkinson's disease (PD) patients with good safety and tolerability. Beyond its neurological benefits, emerging evidence suggests that melatonin may exert cardioprotective effects, which could be relevant in the context of sudden unexpected death in PD (SUDPAR). PD is associated with high mortality rates, and autonomic dysfunction and cardiac abnormalities may play a role in SUDPAR.

Objective: Patients with uncontrolled epilepsy are at high risk for sudden unexpected death in epilepsy (SUDEP), which likely results from a pathological, seizure-induced disruption of vital physiological systems. The objective of this study was to characterize seizure-induced physiological dysfunction leading to death in SS rats and determine the importance of time of day of seizures regarding mortality rates.

Tonic-clonic seizures (TCSs), which present a significant risk for sudden unexpected death in epilepsy (SUDEP), require accurate detection to enable effective long-term monitoring. Previous studies have demonstrated the advantages of multimodal seizure detection systems in reliably detecting TCSs over extended periods.
