Aortic wall degeneration late after ascending aortic wrapping.

Little is currently known about the pathological changes after aortic wrapping. This study investigated long-term outcomes including pathological findings after ascending aortic wrapping. Of the 30 patients who underwent aortic wrapping from 2004 to 2012, redo surgery was later performed in six patients. All the patients underwent ascending aorta replacement because of safety issues associated with aortic cross clamping due to adhesion. Pathologically, the tunica media had been lost and replaced by foreign body granulation. The wrapped aorta manifested severe erosion late after surgery. The wrapped aorta will likely be replaced in a redo surgery.

Introduction: Management of patients with large aortic arch aneurysms who are considered high risk for frozen elephant trunk technique have been challenging, especially when they have a dilated ascending aorta (AA) that precludes total endovascular branched repair (arch BEVAR). A viable option in our armamentarium is wrapping of the AA (AW), and zone 0 Ishimaru TEVAR.

BACKGROUND Acute intramural hematoma (IMH) of the ascending thoracic aorta and aortic arch is a life-threatening condition, particularly in elderly patients with comorbidities, due to its risk of progression and rupture. Unlike aortic dissection, IMH lacks an intimal tear, influencing both clinical presentation and treatment strategy. This report describes a 74-year-old hypertensive woman with type A IMH and a penetrating atherosclerotic ulcer (PAU), managed with a hybrid surgical approach that combines external Dacron wrapping of the ascending aorta and endovascular stenting of the aortic arch with in-situ fenestration of the supra-aortic arteries.

Methods: In this double-blind experimental study, 21 Wistar albino male rats (mean weight: 290 g; range, 280 to 300 g) were divided into three groups: the sham group (n=7), the abdominal aortic aneurysm (AAA) group (n=7), and the AAA-edoxaban group (n=7). Edoxaban 10 mg/kg was given to the AAA-edoxaban group by oral gavage daily for 30 days.
