Comminuted intra-articular fractures of the metacarpophalangeal joint (MCPJ) of the thumb are difficult to treat because of challenges with anatomical fixation. The pins and rubbers traction system (PRTS), described by Suzuki et al., is a minimally invasive technique that enables early range-of-motion training while maintaining joint congruency. PRTS exhibits advantages relative to other dynamic external fixation techniques due to its simplicity, low cost and compact design. It has been used mainly in treating primarily proximal interphalangeal joint (IPJ) fractures. We present two patients where we used a PRTS for comminuted intra-articular fractures of the thumb MCPJ with good outcomes. We temporarily pinned the carpometacarpal joint (CMCJ) in maximum abduction to avoid skin irritation from the ulnar aspect of the axial traction wire in thumb adduction. Level V (Therapeutic).

Surgical treatment of Wassel type Ⅵ radial polydactyly with convergent deformity is technically challenging. We found that 17 of our cases exhibited cartilaginous connections between the bases of the radial and ulnar thumb's metacarpals. To better assess the anatomic features and guide surgical design, we used X-ray and MRI imaging to characterize the adduction of the ulnar metacarpal, radial subluxation of the metacarpophalangeal joint, narrowness of the first web, and thenar muscle distribution.

Arthrosis of the trapeziometacarpal joint of the thumb is a predictable sequelae of ligament laxity. A new technique of tendon interposition arthroplasty with ligament reconstruction using the flexor carpi radialis tendon for a painful arthritic trapeziometacarpal joint of the thumb is described. Twenty-one patients had 25 operative procedures; 14 were women and seven were men.

Background: In rhizarthrosis the saddle joint of the thumb is affected. Occupational therapy after interposition arthroplasty is of particular importance and a key factor for a successful outcome. Orthosis use and/or the timeline of using the orthosis is still under debate.
