Unlabelled: Intervertebral disc degeneration is the leading cause of low back pain, imposing significant burdens on patients, societies, and economies. Advancements in regenerative medicine have spotlighted extracellular vesicles as promising nanoparticles for intervertebral disc degeneration treatment. Extracellular vesicles retain the potential of cell therapy and serve as carriers to deliver their cargo to target cells, thereby regulating cell activity. This review summarizes the biogenesis and molecular composition of extracellular vesicles and explores their therapeutic roles in intervertebral disc degeneration treatment through various mechanisms. These mechanisms include mitigating cell loss and senescence, delaying extracellular matrix degeneration, and modulating the inflammatory microenvironment. Additionally, it highlights recent efforts in engineering extracellular vesicles to enhance their targeting and therapeutic efficacy. The integration of extracellular vesicle-based acellular therapy is anticipated to drive significant advancements in disc regenerative medicine.
The Translational Potential Of This Article: Existing clinical treatment strategies often fail to effectively address the challenges associated with regenerating degenerated intervertebral discs. As a new regenerative medicine strategy, the extracellular vesicle strategy avoids the risks associated with cell transplantation and shows great promise in treating intervertebral disc degeneration by carrying therapeutic cargo. This review comprehensively examines the latest research, underlying mechanisms, and therapeutic potential of extracellular vesicles, offering a promising new strategy for intervertebral disc degeneration treatment.

Objective: This study aims to investigate the relationship between preoperative cervical intervertebral foramen width and area and the persistence of postoperative pain in patients diagnosed with cervical spondylotic radiculopathy (CSR).
Methods: Patients were divided into two groups, based on their pain relief at the 6-month postoperative follow-up: the pain relief group and the persistent pain group. We compared various parameters, including age, sex, body mass index (BMI), duration of symptoms, preoperative Japanese Orthopedic Association (JOA) score, Neck Disability Index (NDI) score, postoperative ratio of disc space distraction, preoperative width of the intervertebral foramen (WIVF), and area of the intervertebral foramen (AIVF) between the two groups.

Background And Objectives: Intervertebral disc degeneration (IVDD) is a complex condition that necessitates the development of novel therapeutic strategies. The objective of this study was to investigate the therapeutic potential of Fuzi decoction (FZD) in the treatment of IVDD by examining its bioactive components, target genes, molecular interactions, pathways, and therapeutic efficacy.
Methods: Bioactive ingredients with an oral bioavailability (OB) of ≥ 30 % and drug likeness (DL) of ≥ 0.

Case: A 34-year-old man presented at our hospital with knee collapse. Magnetic resonance imaging (MRI) revealed posterior compression of the dural sac by a lumbar epidural lesion; however, a diagnosis could not be reached. Gadolinium (Gd)-enhanced 3-dimensional MRI (3D-MRI) clearly delineated the morphology, enabling us to make a preoperative diagnosis of posterior epidural migration of the lumbar disc fragment (PEMLDF).

The skeletal system, composed of bones, muscles, joints, ligaments, and tendons, serves as the foundation for maintaining human posture, mobility, and overall biomechanical functionality. However, with ageing, chronic overuse, and acute injuries, conditions such as osteoarthritis, intervertebral disc degeneration, muscle atrophy, and ligament or tendon tears have become increasingly prevalent and pose serious clinical challenges. These disorders not only result in pain, functional loss, and a marked reduction in patients' quality of life but also impose substantial social and economic burdens.
