Lymphogranuloma venereum (LGV) is a sexually transmitted infection typically caused by serovars L1-L3 of . These serovars are tissue-invasive with a preponderance for lymphatic spread and can be acquired via unprotected oral, anal, or vaginal sex. We present the case of a 23-year-old with a prior history of syphilis admitted with four weeks of progressively enlarging painful right cervical lymphadenopathy. Extensive testing, including oropharyngeal swabs and microbiological testing, did not yield a diagnosis, nor did extensive pathological and microbial testing of the lymph node biopsy tissue. Serological test of lymphogranuloma venereum revealed positive L1 serovar antibodies. The patient had a complete resolution of his symptoms after three weeks of doxycycline therapy.

Proctitis refers to inflammation in the rectum and may result in rectal bleeding, discharge, urgency, tenesmus, and lower abdominal pain. It is a common presentation, particularly in genitourinary medicine and gastroenterology, as the two most common causes are sexually transmitted infections and inflammatory bowel disease. The incidence of infective proctitis is rising, particularly amongst high-risk groups, including men who have sex with men, those with HIV seropositive status, and those participating in high-risk sexual behaviours.

Background: Infection with Chlamydia trachomatis (CT) can have distinct clinical presentations, such as trachoma, or lymphogranuloma venereum (LGV). Certain populations are at greater risk for LGV acquisition and transmission, which requires a longer duration of therapy than other urogenital CT sexually transmitted infections (STIs). Commercial assays are not available in the United States to distinguish LGV from non-LGV serovars.
